the Continuum of Care (CoC) Grant Agreement in the amount of $241,673.00 to

2020; and authorize the HHSA Director or Deputy Director to approve and execute
the grant agreement and any non-monetary amendments thereto, and any other documents related
to the grant.

BACKGROUND/SUMMARY:
CSWD
has been successful in receiving continued funding to support the Helping Hands
Program for the Continuum of Care (CoC) permanent supportive housing to our
homeless community. This program has been funded by HUD the last 7 years
and has served approximately 11-14 individuals and or families annually with
permanent housing. San Benito County is in need of additional resources and
facilities to adequately serve homeless individuals and families. Currently,
CSWD operates through a contractor the year-round HOME Resource
Center and to Homeless Winter Shelter for homeless families during
the winter months of December-March. Last year, CSWD served 24
homeless families.
Under this HUD
grant, we will provide housing shelter by “master leasing” apartments
and/or homes to serve unaccompanied homeless individuals and/or homeless
families. Community Services Development Corporation (CSDC) has been contracted
to provide all aspects of the leasing component of the grant. In addition, CSWD
provides case management and supportive services to all enrolled
participants.
