BACKGROUND/SUMMARY:
The Whole Person Care (WPC) Pilot Program and creation of the California Small County Collaborative (CSCC) was approved by your Board on May 23, 2017. As a follow up item to the creation of the CSCC a Memorandum Of Understanding (MOU) was established and subsequently amended to delineate the responsibilities of the participating counties, establish a cost sharing ratio and confirm the agreed upon financing stream for the intergovernmental transfers (IGT).
Since that time Plumas County has withdrawn from the CSCC and the WPC pilot project. Their termination letter is attached. The two remaining counties, San Benito and Mariposa have agreed to write a new MOU with same purpose as previously mentioned and to acknowledge the withdrawal of Plumas County.
This MOU will supersede the previous MOU and amendments.
