CONSENT AGENDA

BACKGROUND/SUMMARY:

Under a previous Board-approved contract, San Benito County Public Health Services (PHS) currently receives grant funds from the California Department of Public Health (CDPH) for PHS’s Immunization Assistance Program (IAP).

CDPH issues this grant to local jurisdictions to increase immunization (IZ) rates for children through outreach and education activities targeting medical providers, childcare providers, elementary and secondary schools, and other child-serving populations. PHS provides IZ services to Medi-Cal and uninsured children at clinics held once a week (twice a week during the summer), and expanded its IZ services to uninsured and underinsured adults in San Benito County in 2017. H&HSA is responsible for IAP services in San Benito County.

H&HSA is requesting authorization for the agency director to sign the grant agreement with CDPH. Funding will be $36,250 per FY for a total of $181,250 over the grant period.  There is no impact to the County General Fund. These funds are included in the adopted FY2017/18 budget.
