Approve the Continuum of Care (CoC) Grant Agreement in the amount of $237,518 to provide housing, support services and case management to eligible homeless individuals and families, for the period of April 1, 2016 through March 31, 2017; and authorize signing authority to the HHSA Director or Deputy Director to approve and execute the grant agreement (Contract #CA1072L9T061503), and any amendments and other documents related to the grant with the US Department of Housing and Urban Development for the purposes of this grant.

BACKGROUND/SUMMARY:
CSWD has been successful in receiving continued funding to support the Helping Hands Program for the Continuum of Care (CoC) permanent supportive housing to our homeless community. This program has been funded by HUD the last four years and has served approximately 11-14 individuals and or families annually. San Benito County lacks the resources and facilities to adequately serve homeless individuals and families. Currently, CSWD operates two emergency temporary shelters to homeless individuals (Warming Shelter) and to Homeless families (Winter Shelter) during the winter months of December-March. Last year, CSWD served 30 homeless families and 56 homeless individuals.
According to the 2015 Point-in-Time Census Report, there are 651 homeless individuals in San Benito County. Specifically, we will provide housing shelter by “master leasing” apartments and/or homes to serve unaccompanied homeless individuals and/or homeless families. Community Services Development Corporation (CSDC) has been contracted to provide all aspects of the leasing component of the grant. In addition, CSWD provides case management and supportive services to all enrolled participants.
This grant has been included in the FY 16/17 recommended budget.
